Cycles and breaks

Breaks are how tolerance is kept at bay with any mu agonist, and dermorphin builds tolerance more slowly than morphine but still builds it. Its history is short, placed courses for a defined episode of pain rather than ongoing use. Where pain is ongoing, the pain team's whole job is rotating and spacing what is used so that no single receptor pathway is worn down. That planning is clinical work and sits with them.
